Symptom
You having an issue with your SAP system and you want to report the issue to SAP for further investigation and helps.
Solution
1. Login to the SAP support portal using your OSS ID.

2. Under the Help & Support > Report a Product Error. At this page, you need to select the SAP system affected. You can choose from your recently used systems (if someone just recently raised the OSS) or you can search your system.
3. The next page is the Prepare Solution Search. At this page, you need to provide the keyword of the issue that you encountered on your SAP system. Let say as an example, your SAP system is having slow performance when executing transaction code IW47. You can put the Search term as IW47. Additional to this, you will need to know the Component of the reported issue. You can refer to this post to find out the correct component for your reported issue.
4. The next page is the Find Solution. This page is actually giving you some SAP Note which may be can help you to figure out the issue before you continue to escalate to SAP. If you do not find any SAP Note that help you to solve the issue, you can continue by clicking the Create Message button.
5. This page is where you create and enter the message information to the SAP. You will need to write a complete, understandable information to SAP. The message should have affected users, downtime, error re-produce steps and etc. You can save the message if you plan to send on later time or choose Send Message for immediate action.
6. Once sent, you will received the OSS message number which can be your reference when you want follow up or update the message to or from SAP.
7. If you want to search your OSS message or others, you can go to step 1 address link and choose Search Message. Fill in the OSS message number and press Search.
